Make a meaningful impact by supporting adults on their journey to wellness! We’re seeking a compassionate Mental Health Clinician to provide care coordination counseling and recovery-focused services for adults living with serious mental illness (SMI) including those with co-occurring disorders. This is an outpatient role where you’ll work collaboratively to help clients achieve stability and independence.
What You’ll Do:

Deliver individualized and group counseling crisis intervention and case management.

Complete assessments and create personalized service plans with clear goals.

Coordinate care across programs including substance use treatment to ensure continuity.

Connect clients to vital resources such as housing employment and community support.

Maintain accurate documentation and progress notes in an electronic health record system.

Participate in team meetings trainings and community events to enhance client care.

Advocate for clients and collaborate with community partners to support recovery.
What We’re Looking For:

Master’s degree in Social Work Counseling or Marriage & Family Therapy (or equivalent).

Registered or eligible with the California Board of Behavioral Sciences (BBS).

Experience working with adults with SMI and co-occurring disorders.

Strong understanding of recovery principles cultural competence and trauma-informed care.

Excellent communication skills and ability to work collaboratively.

Valid driver’s license CPR certification and TB clearance.

Bilingual in Spanish preferred.
