Job Overview
We are seeking a compassionate and experienced Mental Health Clinician to support adults receiving behavioral health and recovery-focused services. This role provides client-centered clinical support care coordination assessment counseling crisis response service planning and linkage to community resources.
The ideal candidate has experience working with individuals impacted by serious mental health conditions housing instability co-occurring substance use concerns justice-system involvement and other complex life circumstances. This position requires strong clinical documentation skills cultural humility sound judgment and the ability to collaborate with multidisciplinary teams and community partners.
Key Responsibilities
- Provide ongoing care coordination clinical support counseling rehabilitative services and client advocacy.
- Complete client assessments intake documentation service plans progress updates reviews and related paperwork.
- Work with clients to identify goals barriers support needs and appropriate interventions.
- Support clients in accessing services such as housing employment vocational support food assistance benefits medical care behavioral health services and other community resources.
- Coordinate with internal teams and external providers to support continuity of care for clients receiving multiple services.
- Provide crisis intervention and de-escalation support as needed.
- Monitor client progress and update care plans based on changing needs.
- Participate in care coordination meetings client consultations case reviews and team discussions.
- Maintain communication with community partners case managers conservators treatment providers and referral agencies.
- Prepare written communication as needed including letters updates court-related documentation and service summaries.
- Maintain accurate timely and detailed documentation in an electronic health record system.
- Track daily client contacts progress notes referrals and follow-up activities.
- Attend relevant trainings community meetings and events related to evidence-based practices and behavioral health services.
- Seek guidance from supervisors or clinical leadership when complex client needs or safety concerns arise.
- Perform other duties as assigned.
Qualifications
- Experience providing mental health services behavioral health support counseling care coordination case management or rehabilitative services.
- Knowledge of recovery-oriented trauma-informed and solution-focused service approaches.
- Familiarity with mental health diagnoses including DSM-5 diagnostic categories.
- Experience supporting adults with serious mental illness co-occurring disorders homelessness justice-system involvement or complex psychosocial needs.
- Ability to complete biopsychosocial assessments service plans progress notes and related client documentation.
- Understanding of confidentiality professional boundaries mandated reporting and ethical service delivery.
- Strong communication organization problem-solving and documentation skills.
- Ability to work effectively with culturally diverse populations including Latino African American LGBTQIA+ and other underserved communities.
- Proficiency with electronic health records Microsoft Office databases and standard office equipment.
- Ability to exercise independent judgment while following program policies procedures and regulatory requirements.
Preferred Skills
- Experience facilitating individual or group counseling services.
- Knowledge of community resources in housing employment benefits behavioral health substance use recovery and social services.
- Experience working in a multidisciplinary behavioral health or community-based setting.
- Bilingual English/Spanish skills are a plus.
Work Environment
This role may include office-based services community-based coordination client meetings documentation team collaboration and participation in trainings or outreach events. The position requires professionalism flexibility and a strong commitment to serving individuals with complex behavioral health and social service needs.

Pay
- $32-38/hour
