About the Role:
You’ll provide outpatient care coordination and therapeutic services for adults (18+) living with severe mental illness (SMI) including those with co-occurring disorders (COD). This role combines clinical expertise with compassion to help clients achieve stability and independence.
What You’ll Do:
Deliver biopsychosocial assessments counseling (individual & group) and crisis intervention
Develop personalized service plans and coordinate care across multidisciplinary teams
Connect clients to community resources like housing employment and vocational support
Maintain accurate documentation in EHR systems and participate in care review meetings
Collaborate with our SUD program for continuity of care
Attend trainings and community events to stay current on evidence-based practices
What We’re Looking For:
Education: Master’s degree or higher in Social Work Counseling or Marriage & Family Therapy
Experience: Graduate-level internship with SMI clients or 1–2 years post-master’s experience with SMI/COD populations
Licensing: Registered or eligible with CA Board of Behavioral Sciences (BBS)
Skills: Strong clinical judgment cultural competence and ability to work with diverse populations
Other Requirements: Valid CA driver’s license auto insurance CPR certification TB test
Preferred: Bilingual (Spanish) – bilingual candidates will complete a competency test
