One-of-a-kind human services company offering services for people with intellectual and developmental disabilities home care for seniors as well as education vocational training and job placement for people of all ages and skill levels. We are also dedicated to helping end homelessness by providing shelter and housing assistance to those we serve.

 

Program provides outreach temporary shelter via Hotel Voucher transportation and Housing Location services to clients that are experiencing homelessness. We are contracted through the County of San Diego Health and Human Services Agency within the jurisdiction of the Housing Authority of the County of San Diego.

 

Through organized county outreach events and referrals we enroll eligible clients that are experiencing homelessness and provide shelter. Our Housing Locators will work diligently with clients to identify and apply for housing opportunities. This includes locating affordable units completing applications preparing clients for landlord interviews and attending landlord interviews with the clients.

 

Pay: $27.00/hour

Location: Escondido Office for training and meetings as assigned; travel throughout Poway and Santee

Schedule: Monday/Friday 8am-5pm (Full Time)

 

This position is responsible for documenting all services provided ensuring case notes are entered into the participant’s electronic record in real-time.

 

  • Have knowledge of the homeless community and County demographics

  • Implement and apply regulations to daily duties

  • Exercise attention to detail

  • Organize prioritize and complete a large volume of work within strict time deadlines

  • Build and maintain positive relationships with landlords and property management companies serving low income families. Represent and promote the program throughout the community by educating them about the housing needs of identified program participants and the importance of supportive housing.

  • Assist families and individuals to establish and meet housing goals and identify appropriate steps towards achieving goals including modifying goals as needed and evaluation of progress. Coordinate and collaborate effectively with other service providers as needed to assist participants to reach their goals.

  • Promote values of self-sufficiency and empowerment throughout work with participants. Aid in the development of resource materials specific to neighborhoods/cities/counties where the families reside. This may include community-based referrals housing and employment advocacy implementation of service plans educating participants about budgeting and financial management and employment. Support client employment search and refer participants to appropriate resources

  • Participate in various program meetings including case conference meetings and housing plan meetings in collaboration with other service providers. Attend all agency staff meetings and trainings as well as department meetings and case conferences.

  • Maintain precise and accurate documentation of case management services including participant files and entries into participant databases.

  • Maintain participant confidentiality and HIPAA requirements; maintain program standard of documentation of case management services; keep accurate records files correspondence and statistics. Prepare reports and presentations as required.

  • Maintain thorough and accurate progress notes files and correspondences while maintaining confidentiality of tenants staff and organizational information always and exercise appropriate boundaries with tenants.

 

Qualifications

Bachelor’s Degree in Human Services field preferred or equivalent skills and experience

Intermediate computer skills in Microsoft Office Email and Data Entry

Strong communication skills both written and verbal

Driver’s License with good driving record
